CRIS Pathway
The aim of this pilot service is to support patients to remain at home following contacting their GP / 111 / Community Pharmacist. If a Community Pharmacist is unable to advise or support with sub-acute clinical need following a Pharmacy First referral or feel patient may have deteriorated since contact with GP they can be referred to UCCC as in the flow chart (see links below)
Community Pharmacists can complete the referral in PharmOutcomes and choose the completion option that indicates an onward referral has been made to CRIS. This information is then received by the GP clinical system.
